DOCUMENTS required for admission at "GLSIHOME"
At the time of registration/ booking the applicant should furnish the following documents in photocopy & original (only for verification) to the authority:
- 1. Proof of Age.(aadhaar, voter, passport etc).
- 2. Proof of Address (aadhaar, Voter card, passport etc).
- 3. Report of some routine the tests as advised by "GLSIHOME".
- 4. Full name, surname, full address and contact telephone number(s) / mobile no. of his/ her spouse, children, Guarantor , near relative, close friend and applicant's relationship with such person.
- 5. All previous medical records, medicine list, name & contact of the treating doctor (if any).
- 6. AFFIDAVIT as per FORMAT of “GLSIHOME” (will be provided).
- 7. Properly filled application form.
ELIGIBILITY of becoming a resident:
- 1. Any single person , spouse, brother / sister can be a member of ‘GLSIHOME”.
- 2. Applicant should be at least 50 years of age if single.
- 3. In case of couples at least one of the partners should have attained the age of 50 years.
- 4. Pre-admission medical assessment.
- 5. For Membership / registration - one should be at least 50 years and submit Rs. 1000 (one thousand only) + Rs.150
Advantage of prior Membership / registration:
- You will get free membership of the club “GLSIHOME VISTA”.
- Will be eligible for short stay at "GLSIHOME".
- Will get a priority if you want to become a permanent resident.
- Will be entitled to attend various programmes organised by "GLSIHOME" authority.
